There are two fees, paid to two different places. We do not publish a price list here. The practice evaluation fee is quoted to you in writing before you book, so you know it before you commit to anything, and the state sets its own registry fee on its own schedule.
The practice evaluation fee
Charged by the practice for the physician's time and judgment: the examination, the review of your history, and a decision made on the merits of your case. You are told this fee in writing before you book, and you are never asked to commit to anything before you know it.
The state registry identification card fee
Set and collected by the State of Florida, not by this practice. Because the state sets it and can change it, we link the official source rather than reprint a figure that may be out of date by the time you read it: the state's own fee page.

Why the two fees stay separate
A certification involves two different payees doing two different jobs. The practice fee pays for the physician's evaluation — the examination, the history review, and the clinical judgment about whether to certify. The state fee pays for the registry identification card and is set and collected by the State of Florida. We never blend them into a single “total cost,” mark up the state fee, or collect it on the state's behalf. If the state changes its fee, that decision is the state's; we link its official page rather than reprint an amount that could be out of date by the time you read it.
Cancellation and rescheduling
You are paying for the physician's time and judgment — not for an outcome. Our cancellation and rescheduling terms are given to you in writing along with the fee, before you book, so there is nothing to discover after the fact.
If you need to move an appointment, contact us as early as you can. Rescheduling windows exist so that a slot you release can go to another patient waiting for an evaluation.
Insurance and payment
Certification evaluations are typically self-pay, and we do not bill insurance for them. Health plans generally treat medical marijuana certification evaluations as non-covered services — but coverage is a plan-specific question, and we cannot verify benefits for you. If it matters to your decision, ask your insurer directly. The state registry fee is simpler: it is paid to the state and is not an insurance matter at all. What we can commit to is clarity — you will always see the practice fee, its effective date, and the cancellation policy before you commit to an appointment.
