Who can apply

Adults living in Florida — year-round or seasonally — can request an evaluation with a state-qualified physician. Two things must line up before anyone holds a card: a medical condition that qualifies under Florida law, and the physician's independent judgment that certification is appropriate for you.

A listed diagnosis supports your request — it never guarantees certification. That decision belongs to the physician alone, made only after a real evaluation.

The three-step path

One path, two systems. Almost every delay — and every scam — in this space comes from blurring the line between the physician's role and the state's role.

1. 01PhysicianPhysician certificationMeet a Florida-qualified physician for an evaluation — first certifications require an in-person physical examination. If the physician certifies you, the certification is entered into the state's Medical Marijuana Use Registry. If the physician declines, the path ends there, and under our cancellation policy you are not charged for the evaluation.
2. 02You + the stateRegistry applicationWith an active certification in the registry, you complete the state's own application: identity and residency documentation, a photo, and the state fee — paid to Florida, never to the practice.
3. 03StateState ID cardThe state reviews your application and issues the registry ID card on its own schedule. Only the state issues cards, only the state controls timing, and an active card is required to purchase from licensed treatment centers.

What the physician decides — and what the state decides

We are a physician practice. We are not the Medical Marijuana Use Registry, we do not represent the State of Florida, and nothing we say can change a state decision or timeline.

Documents to bring

• A valid Florida photo ID. Seasonal residents should bring the documentation the state accepts for part-time residency.
• Medical records related to your condition, if available — they help the physician review your history.
• A current medication list and the names of your treating clinicians.
• Payment for the practice evaluation fee.

After certification, the state's application asks for its own identity and residency documents plus the state fee. If the physician's office needs anything else, you will be told before your visit — our scheduling form never asks for medical details.

What it costs — two separate fees

Our new-patient evaluation fee is $149 (effective July 23, 2026), charged by the practice for the physician's evaluation. The state's registry ID-card fee is $75 (effective July 23, 2026), set and collected by Florida — never by us.
